You should take out the wine bottle, and replace it with a second pistol. That would be uber-badass. And if you tilt the head slightly away from the camera, it removes the need for facial features. Also, the hand holding the bottle is at an odd angle, around 115 degrees. If it were lowered to 130 it...

Realtime? Naw, that can get old. In video games, timing is everything. Although, this rule is not set in stone.
