Closer inspection of the gif has made clear that, in fact, the entire body is hollow.

In UT2004 they at least had bone reduction functions that would reduce vertices to a point.

It's either a wonky still that we have from that video or a placeholder. By the looks of previous trailers and gameplay, there is a decent amount of possible dismemberment in the game. No developer, let alone one with as high a graphical standard as Naughty Dog, would be stupid enough to let you blow off a zombie's limbs and then show you that it is completely hollow inside.

Indie games do that to garner attention, when triple A games do that, it's usually a bad sign.
