One (twoish) thing I'd like to see is dynamic cloud movement - that is, simply, that clouds will move in the sky and cast respective shadows on the ground. Instant-killing aware opponents on Insane
Is it possible to perform a Backbreaker/Tracheotomy/Backstabber on an aware opponent on Insane difficulty? By "aware" I mean you're in combat with them and you flip over them, as opposed to sneaking up on them.

On Normal (or Hard, or whatever) it seemed to me that the chances of hitting a direct knife throw were like 80%, and with a flip throw about 50-60%.
