It's from PatSilverFox's Shrine map, Anton added it to the Main Menu Mod, I believe.

Which map is that from?
It's a pretty new map called "Shrine". That's the menu button if you load the level. Update the Menu Mod and it will be the first custom map (for now).

I really like this custom sword Masamune by aaorris.
Using it along with the "Bola" it's a really great looking combo!
